Health Office Updates for School Year 2017/2018

Up-to-Date Immunization Records Needed for K-12:  Colorado law requires students who attend public kindergarten-12th grade schools to be vaccinated against many of the diseases vaccines can prevent.
The Health Office has mailed outa Notice of Immunization Requirements for those students that we do NOT have complete records for.  PLEASE provide the Health Office with your child’s updated information as soon as possible.
 Colorado rule requires that students entering kindergarten receive their final doses of DTap, IPV, HepB, MMR and Varicella.  Students must receive 1 does of Tdap vaccine for 6th grade entry, even if they are under 11 years of age.  The number, timing and spacing of the required vaccine doses is set by the Centers for Disease Control and Prevention’s Advisory Committee on Immunization Practices (ACIP).  You can view parent-friendly versions of the current ACIP vaccine schedules for children 0-6 years of age at:
Please take your students updated vaccine record to school every time he or she receives a vaccine.  If your student cannot get vaccines because of medical reasons, you must submit an official Immunization Medical Exemption Form to your school.  Singed by a health care provider licensed to give vaccines.  You can get the from at:  If you choose not to get your student vaccinated according to the current ACIP schedule for religious or personal belief reasons, you submit a non-medical exemption to your school.  Non-medical exemptions must be submitted every year.  You can pick up this form in the school Health Office or you can get a downloadable version online at:

Parents of incoming 6th Grade Students for our 2017-2018 School Year!
T-dap is a required Immunization for 6th grade and is an important step in protecting your child from Tetanus, Diphtheria and Pertussis (whooping cough). Immunization Exemptions for School Year 2017-2018
Students in grades K-12 claiming a non-medical exemption (personal belief and religious) must submit one annually. These exemptions expire June 30 each year. Parents of students in preschool must also submit non-medical exemptions, these exemptions expire when the next vaccines are due or when the child enrolls in kindergarten. You can find this Non-Medical (personal belief and religious) Exemption Form below or on the Colorado State’s Website. The Colorado Immunization website is located at:
